To help us accommodate your busy schedule and get you in and out of our office as quick as we can, please print off and fill out the below forms ahead of your exam.
Online Payment Center
We are now accepting payments online for any balances due. If you would like to make a payment online, please click the button below! It will take you to a third party web site that we are using to process these payments.
Patient Portal
Please login in to the Patient Portal to access your online medical records, prescriptions and see if you have any outstanding balances. This service is no charge to any James Eyecare & Optics Gallery patient and your login should have been provided when you checked out at your last appointment.

Insurance Info
We accept most major medical insurance plans and vision benefit plans, and you will find a list below. However, the best way to find out your benefits is to call your insurance company and ask if James Eyecare & Optics Gallery, Dr. Julie Dolven or Dr. Kathryn Hutchins is on your provider list.
